Google Forms

Module 4 Outline


1. Introduction:

This module is designed as an independent course study for material on Google Forms. The goal of this module is to help you create self-grading quizzes, social-emotional check-ins, and reflections. As you proceed, please read all instructions, watch any video material and check out all attachments for module information. Email the instructor (pshoemaker@lamar.edu) with any questions on module content.

c. Website showing examples of Social -Emotional Check -ins.


3. Learners will be able to:

a. Create a Google Form

b. Customize the theme.

c. Add an image header

d. Create a self- grading quiz

e. Import the grades into Google Classroom


4. Reflection:

Learners will answer questions verbally using Flipgrid

a. What was your favorite part of this module?

b. How will using your Google Site benefit you and your students?

c. How would using Google Sites as an eportfolio platform benefit your students?


5. To Receive Credit:

To receive credit for this module, you will complete each assignment, including the reflection. You will attach the link to each Google Form to the appropriate assignment. Once you are finished with this module, you will receive 2 PD credits in Eduphoria.

a. Assignment 1: Attach the link to the Google Form to the assignment and insert form onto the digital

lessons page of your Google Site..

i. A picture header

ii. A multiple choice question

iii. A true-false question

iv. Each question is required and assigned point values

v. Grades can be seen immediately

b. Assignment 2: Attach the link to the Google Form to the assignment and insert form onto the digital lessons page of your Google Site.

i. At least one linear scale question

ii. At least one multiple choice question

iii. Short answer question

iv. Should collect email addresses
